Registering with the surgery
To register with the surgery:
- fill out an online registration form
- alternatively, you can register by printing off and returning a completed copy of the PRF1 New Patient Registration Form to the Surgery, please note if you are unable to print your own form then a copy can be collected from reception.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
The NHS requires that every patient is allocated a named accountable GP. We will inform you either at the reception desk or by text message who your named GP is. This is for administrative purposes only and you retain the right to see any of our GPs. You will still be able to book an appointment with the GP of your choice.
