If you are unwell and cannot work, you may need a fit note (sometimes called a sick note) for your employer.
If you’ve been off sick for more than 7 days
To request a sick note:
- use our online form
- call us on 01723 515666 during our opening hours
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You may need to complete a SC2 Self-Certification form when you return to work, these are available from your employer or click here to download a copy.
